Monday-Friday 8am-5pm (Staff on-call after hours)

Referrals by central telephone intake required; home visits scheduled for assessment
Eligibility requirements
Must be 60 years old or over with psychiatric disability or substance abuse disorder. Client must meet State defined criteria.
